Clients requesting access to Home and Community Care (HACC) are visited at home by a member of the Regional Assessment Service (RAS).
A holistic wellness based assessment is completed in partnership with the client and his or her carers with a focus on reablement.
If the client requires support, the RAS identifies service providers with the capacity to assist. Once the client has chosen their preferred service provider the RAS sends the referral assessment and support plan to the service provider.
