Community packages (ComPacks)
ComPacks provides short-term support with daily activities for people being discharged from public hospital. This program is only available in New South Wales.
The aim of ComPacks is to facilitate the safe and early discharge of people from hospital by providing access to a short-term package of care.
We help to identify what support you need to:
- regain your independence
- prevent re-admission to hospital.

How it works
One of our ComPacks case managers will work with you over a six-week period, developing strategies to address short-term functional challenges. We can also set up access to longer-term support if you need it.
This can include:
- personal care
- domestic assistance
- social support
- in-home respite.
How to access ComPacks
You will be referred to our ComPacks program by your hospital. It’s available to people of any age.
Eligibility is based on your need for immediate access to case management and community support that can't be access through other channels.
